How Insurance Covers Addiction Treatment

The Affordable Care Act (ACA) classifies substance use disorder treatment as one of the ten essential health benefits that most insurance plans must cover. This means that if you have health insurance through your employer, the marketplace, or Medicaid, your plan is required to include some level of coverage for addiction treatment services.

Additionally, the Mental Health Parity and Addiction Equity Act requires insurance companies to provide the same level of benefits for mental health and substance use disorders as they do for medical and surgical care. This means your copays, deductibles, and visit limits for addiction treatment cannot be more restrictive than those for other medical conditions.

Coverage typically includes medically necessary services such as detoxification, inpatient rehabilitation, outpatient counseling, medication-assisted treatment, and behavioral therapies. Ask about your deductible, copay or coinsurance amounts, any pre-authorization requirements, and whether the treatment center you are considering is in-network. Alternatively, many treatment centers offer confidential insurance verification. Not having insurance does not mean you cannot access treatment. There are several options available. Many states offer publicly funded treatment programs through SAMHSA block grants, which provide no-cost or low-cost treatment for individuals without insurance. Sliding scale fee programs adjust costs based on your income and ability to pay. Some nonprofit treatment centers offer scholarships or grant-funded programs. You may also qualify for Medicaid, which provides comprehensive addiction treatment coverage. Out-of-network providers do not have a contract with your insurer, so the rates are typically higher and you may be responsible for a larger portion of the bill. However, many insurance plans still provide some level of out-of-network coverage, and the Mental Health Parity Act requires that out-of-network benefits for addiction treatment be comparable to those for other medical services. It is sometimes worth choosing an out-of-network provider if the program is a better clinical fit for your needs.
In the vast majority of cases, no. Federal regulations, including HIPAA (Health Insurance Portability and Accountability Act) and 42 CFR Part 2, strictly protect the confidentiality of substance abuse treatment records. Your employer cannot access your individual medical claims or treatment details, even if they provide your health insurance. The Explanation of Benefits (EOB) sent by your insurance company is directed to the policyholder, not the employer. If you are on a family plan and concerned about another policyholder seeing the EOB, you can contact your insurance company and request that statements be sent to a different address or delivered electronically to a private account.
The cost of addiction treatment varies widely depending on the type of program, location, amenities, and length of stay. Medical detox programs typically cost between $1,000 and $1,500 per day. Inpatient or residential treatment ranges from $5,000 to $80,000 for a 30-day program, with luxury facilities charging more. Outpatient programs are more affordable, ranging from $1,000 to $10,000 for a three-month program. Intensive outpatient programs (IOP) typically fall between $3,000 and $15,000. However, there are many ways to reduce these costs, including state-funded programs, sliding scale fees, scholarships, and financing plans. The cost of not getting treatment is almost always greater, both financially and personally, than the investment in recovery.
