The clinic provides a wide range of services through their Care Coordination Program, including chronic illness and healthcare management for conditions such as HIV, substance use, mental health, and obesity. Clients must be at least 21 years old and have Medicaid in order to qualify for the program.
Clients who enter the Care Coordination Program will be assigned a Care Manager who will help you navigate the program and ensure you get the resources you need. Under the program’s care management service model, clients’ caregivers will maintain open lines of communication with each other and work together to create a comprehensive treatment plan.
While in the program, clients can get assistance with doctor’s appointments, assistance with applying to entitlement programs, and more.
Hunts Point Multi-Service Center believes that stable housing is the foundation for addressing many major challenges. Their housing services include congregate and scattered-site supported housing.
The scattered-site supported housing program is broken into two sections. There’s a United States Department of Housing and Urban Development (HUD) funded program that has 53 units and the Stable Homes to Health program which has 21 units. As part of the scattered-site supported housing program, clients will have a case manager who makes weekly visits. The clinic also has a program called Transform that works with families as they move from shelters to permanent housing.
