Located in Huntsville, Alabama, Huntsville Hospital is an 881 bed hospital that has a transitional care program for individuals who struggle with opioid use disorder. The program was started in response to the high risk of poor outcomes for hospitalized patients with OUD.
The Transitional Care Team is staffed with patient navigators who will provide individuals with case management services and motivational support. Clients will be connected to resources within the community to help support their overall recovery.
Clients may receive assistance getting access to education, vocational training, and housing. Patient navigators will follow up with patients after they’ve discharged and help them connect with the appropriate recovery services to overcome addiction. The Transitional Care team strives to increase the number of individuals who seek outpatient treatment after they’ve completed a hospital stay.
The OUD Moms Outreach initiative at Huntsville Hospital provides education to pregnant mothers about opioid use disorder. The clients will learn about methods that will help them manage children who are born with neonatal opioid withdrawal syndrome so that better outcomes may occur for the family.
