Norfolk Community Services Board is an organization that provides case management for residents of Norfolk, Virginia, who have substance use needs. The organization started in 1969 and currently helps over 9,000 residents. The case management program helps individuals dealing with substance abuse and their families get access to the resources they need.
Some of the resources that a client can be provided are life skills education for independent living and transportation services for supporting the plan of care. Patients will also be guided towards accessing detox, residential and supportive housing services. The goal of the case management program is to improve the lives of clients with co-occurring or substance use disorders.
One of the things I like most about Norfolk Community Services Board is they have case management services specifically designed for women struggling with substance use. They assist pregnant women with getting into the Southeastern Family Project residential treatment program. They also help guide female clients to employment counseling and job placement services, mental health services, and health education. The supportive housing case manager assists in finding temporary housing for individuals enrolled in substance abuse services.
Clients are also assisted in finding employment in order to transition to independent housing within five months. For detox, case managers will assess an individual’s eligibility to receive placement in a facility. The staff will contact multiple facilities and medical professionals to find bed space and conduct discharge planning. Your case manager will also help individuals with avoiding relapse and provide support to assist them with resolving any crises.
