The City of Norfolk Case Management office’s Tidewater Drive Center offers help for substance abuse and mental health treatment. You’ll work together to design a wellness plan that guides you through recovery. Case managers are there to provide support along the way. They also offer a Program of Assertive Community Treatment, abbreviated as PACT. It’s a community service that includes intensive mental health assistance. This program can decrease the chances of hospitalizations and incarceration. They emphasize outreach to build connections and include a multidisciplinary team of experts, like a psychiatrist, case managers, nursing staff, and peer specialists.
There are also resources available, like better coping methods, skill building, and stress and health management. Case managers can help provide the continuum of care you need by making services more accessible.
