Pembroke Hospital is a 120 mental health and co-soccurring substance abuse center for teens and adults in Pembroke, Massachusetts. They specialize in inpatient and partial hospitalization treatment, with a telehealth option available for the latter. If transportation is an issue for you, they can make arrangements so you can receive treatment. This location accepts most forms of insurance.
You’ll work with a diverse team of healthcare professionals at this clinic, but you’ll also connect with social workers and case managers. These people will focus on setting you up for success post treatment while you focus on recovery.
It’s the job of social workers and case managers to ensure you’re meeting your daily needs and can successfully live alone. They also help with care coordination with all of your providers so you don’t miss appointments. Their support reinforces and continues the work started in treatment.
The adult partial hospitalization program (PHP) features family meetings. These sessions are not only a chance for you to connect with family and share what you’ve learned, but it’s also an opportunity for your loved ones to heal themselves. Strengthening family relationships is shown to reduce the risk of relapse. (The PHP can be used as a step down from inpatient care.)
