Project Transition in Chalfont, Pennsylvania, is a mental health treatment facility that offers behavioral health services and support for adults. Their clients are individuals who are struggling to cope with the impacts of a mental health condition, medical diagnosis or dual diagnosis with substance use disorder.

Depending on each patient’s needs, treatment might involve dialectical behavior therapy, psychiatric rehabilitation, a wellness recovery action plan, alcohol focused or addiction focused meetings and medication management. Systems of support services provide crisis prevention, intervention and stabilization help when members find themselves in an overwhelming situation.
