What to Look For in a 12-Step Program

12-Step describes how intensive and how structured care is — how many hours a week, how much medical supervision, and whether you sleep at the facility. It does not describe which therapies you receive, so two programs at the same level can be very different. Level of care should be assigned by clinical assessment, most commonly using The ASAM Criteria, which weighs withdrawal risk, other medical conditions, mental health, readiness to change, relapse history and how safe your home environment is.

The program should be able to say which factors led it to recommend this level for you, rather than defaulting to whatever it happens to sell.

12-Step: Frequently Asked Questions

That should come out of a clinical assessment rather than a preference. The factors that decide it are your withdrawal risk, any other medical conditions, co-occurring mental health needs, your history at lower levels of care, and whether your home environment supports recovery or undermines it. A program that recommends a level without asking about those has not assessed you.
Length varies with the level of care and with the person, and no honest program will quote a fixed number before assessing you. What the evidence supports more consistently is total time engaged in treatment — typically three months or more across all levels combined, with lighter continuing contact well beyond that. Treat any single stage as one part of a longer plan.
Plans that cover mental health and substance use benefits must apply no more restrictive limits to them than to comparable medical and surgical benefits, and substance use treatment is an essential health benefit on marketplace and most small-group plans. Coverage at a given level of care still depends on medical necessity criteria, whether the facility is in-network and how many days the insurer authorises. Verify benefits before you admit.
