They’re the leading provider of affordable behavioral healthcare in California. They particularly target the underserved and socially disadvantaged population including people who are homeless and people with limited access to healthcare. This location is their Integrated Care Center (ICC) in San Francisco which delivers nonjudgmental care for people with diverse health needs.

ICC is situated on Mission Street in the SoMa neighborhood of San Francisco, California. This area is close to Van Ness Avenue and Market Street. It is just a mile northeast of Mission Dolores Park and Southwest Yerba Buena Gardens.

ICC provides holistic support that can help you regain full control of your life from substance use or other addictive behaviors. They offer therapy groups and individual counseling in an outpatient setting. This is geared towards helping you build practical life skills that encourage sobriety and relapse prevention. Group therapy fosters recovery through peer support and community engagement. You’ll be able to practice healthy communication, problem-solving and anger management which are potent tools for a successful sober lifestyle.

ICC also provides medication assisted treatment (MAT) to support those dealing with opioid addiction or stimulant use. ICC also extends support to people struggling with quitting smoking or chewing tobacco via their tobacco cessation therapy.

Additional recovery oriented services available here include nutritional counseling and case management. Nutritional counseling ensures you’re getting the right balanced meals to support your physical healing and boost your overall wellness as you navigate recovery. Case management involves care coordination and linkage to community services that can help sustain your recovery. This includes housing support, employment assistance and transportation services. They even offer onsite charter schools where you can earn a high school diploma or GED.

IV drug users are typically prone to infectious diseases due to sharing needles and other paraphernalia. ICC offers confidential HIV and hepatitis C screening to foster early detection and stop community spread. Supportive counseling and therapy are provided as well to help you recover or manage these infections. TB testing and treatment are accessible as well.

Women in recovery can access various health services here as part of integrated care. This encompasses pregnancy testing, breast examinations and cervical cancer screening. The facility also offers screening for high blood sugar, high blood pressure and high cholesterol. Other services available here are routine medical checkups, dental support and follow-up of chronic and ongoing medical issues.

Levels of Care

12-Step

12 step programs such as Alcoholics or Narcotics Anonymous are support group style programs for individuals who are experiencing alcohol or substance use disorder. At Evoke they use a holistic model of addiction care that includes using 12 step programs. These programs are an integral part of most relapse prevention plans and provide continued support and accountability that support sobriety.

24-Hour Clinical Care

24 hour clinical care provides close medical supervision for individuals in the early stages of recovery. Medical professionals monitor individuals around the clock for complications and provide immediate intervention when needed.

Aftercare Support

Completing a drug or alcohol rehab program shouldn't spell the end of substance abuse treatment. Aftercare involves making a sustainable plan for recovery, including ongoing support. This can include sober living arrangements like halfway houses, career counseling, and setting a patient up with community programs like Alcoholics Anonymous (AA) or Narcotics Anonymous (NA).

Inpatient

Residential treatment programs are those that offer housing and meals in addition to substance abuse treatment. Outpatient Programs (OP) are for those seeking mental rehab or drug rehab, but who also stay at home every night. The main difference between outpatient treatment (OP) and intensive outpatient treatment (IOP) lies in the amount of hours the patient spends at the facility. Most of the time an outpatient program is designed for someone who has completed an inpatient stay and is looking to continue their growth in recovery. Outpatient is not meant to be the starting point, it is commonly referred to as aftercare.

Sober Living Homes

Sober Living Houses (SLHs), aka sober homes or halfway houses, are safe, substance-free, supportive living facilities for those recovering from substance abuse. Ideal for those who've just been through inpatient or outpatient treatment, SLHs are supervised environments with rules that support sobriety, such as curfews, shared chores, and therapeutic meetings. Residents are also often trained on life skills and coping skills to make it easier to transition into society. SLHs also provide a strong sense of community that can lead to the kind of deep and lasting connections with other sober individuals that supports a new, healthy lifestyle.

Adult Program
