The Resource Center Health Home Service takes care of people with dual diagnoses or those who struggle with physical health issues in addition to mental health issues. It allows your caretakers to communicate with each other to provide the best possible care for you.
You will be assigned a Health Home Care Coordinator who can coordinate your medical, mental health and substance abuse care. They can also help you find employment and housing.
If you are a recipient of Medicaid Managed Care Health and Recovery Plan (HARP) then they offer in-home services where a therapist can come to you. A therapist may visit you at home or in your community. This is part of their Community Oriented Recovery & Empowerment program.
